PHQ-9
The Patient Health Questionnaire-9 (PHQ-9) is a 9-item questionnaire used to screen for and monitor the severity of depression. It asks about the frequency of nine common depression symptoms over the past two weeks.
GAD-7
The Generalized Anxiety Disorder-7 (GAD-7) is a screening tool used to assess the severity of generalized anxiety disorder (GAD). It asks about the frequency of seven common anxiety symptoms over the past two weeks.
ASRS-v1.1
ASRS v1.1 is an acronym for the Adult ADHD Self-Report Scale, Version 1.1, a screening tool used to help identify potential attention-deficit/hyperactivity disorder (ADHD) symptoms in adults. It is a self-report questionnaire with 18 questions that asks adults to rate how often they have experienced certain ADHD-related symptoms over the past six months.

The Posttraumatic Stress Disorder (PTSD) Checklist for DSM-5 (PCL-5) is a 20-item self-report questionnaire used to assess the presence and severity of Post-Traumatic Stress Disorder (PTSD) symptoms in accordance with the DSM-5 criteria.
About me
My name is Scott Lawrence and with Over 32 years of experience in the mental health field, I offer a knowledgeable, compassionate, and patient-centered approach to psychiatric care. My journey began as a mental health technician and later as a therapist—roles that provided a strong foundation for understanding the individuals behind the symptoms. In 2006, I became a Board-Certified Psychiatric Mental Health Nurse Practitioner, expanding my ability to support individuals across a wide range of mental health needs.
Over the years, I’ve had the privilege of working with a diverse range of individuals and communities. My background includes Active Duty service in both the U.S. Army and Air Force, roles within the Federal Government and the Veterans Administration, as well as providing care in nursing homes and private practice. These experiences have shaped my understanding of how mental health challenges affect people from all walks of life and at every stage of life.
I specialize in treating complex conditions such as PTSD, depression, anxiety, ADHD, and other challenging mental health concerns—particularly among veterans and individuals who may have been dismissed or overlooked.
One of the guiding principles of my practice is this: every person is worthy of respectful, evidence-based care—and no one is beyond hope.
